You are engaging with the entity of Creative Services Boutique, LLC (copy writing, web design, graphic design, EPK, social media content and other creative and personal services) remote services business

address 1441 Woodmont Ln NE Suite #1291 Atlanta, GA 30318. These terms exist to protect its clients and apply to all bookings, consultations, produced content, documents, files and promotional material.

Client Responsibilities

All clients are expected to do the following:

Provide the required images and documents requested in order to move forward within the timeframe of the target date for the project.

Health Conditions: Clients must disclose any medical conditions or injuries that may affect their massage and/or detox treatment and update us of any changes.

Punctuality: We have a respect for your time and hope you value ours. We ask that clients arrive to appointments and consultations 15 mins before the start of your appointment time in order to start on time.

​

Late Arrivals: We allow a 10-15 min grace period that will also shorten the timeframe of the session. After 15 mins the client has to reschedule and will have a $50 late cancellation fee that must be paid on the date  it is applied.

Pricing:  Fees are based on customized services provided due to the individual request for each client.

​

Methods of Payment: We accept Zelle, cash, Visa, Mastercard, Discover, American Express, and issue invoices for requested services, after payment is received. Deposits or a card on file is  required to secure a booking and move forward with a CSB request.

Respecting Boundaries: We express the  importance of professional conduct and we maintain a respectful environment during sessions. We have the right to end or cancel a session at any time if the technician is uncomfortable or feel endangered.

Consultations:  We ask that you arrive 10 mins before appointment time in order to begin on time. 

​

Cancellation Policy: We require a 24 hour notice for any rescheduled appointments or cancellations, for CSB a $25 fee is applied. Client may be  invoiced for this fee, they agree to make payment and fulfill the obligation for any and all fees issued by invoice or a digital requests. If a missed or cancellation fee is applied and not paid the client will not be able to rebook until it is settled.

Liability Waiver

Liability Waivers are required to completed before any and all services are rendered. Clients acknowledge the risks of treatment and release liability if they fail to disclose relevant health issues. Please check with medical provider for clearance after any diagnose, injury or treatment.

Confidentiality and Data Protection: We handle all client data and information  securely.

The following information is gathered from our visitors to our website: IP address, user account information when applicable, site visitation information, page views, browsers used and browser capabilities and site search queries. We also collect and retain any information submitted to our organizations via the contact and other forms located in the site, scheduling apps, and  consultation submitted paperwork.
